    Failure Mode Effect Analysis (FMEA) Applied to Production Operations

    This course will cover several aspects that contribute to the optimization of production operations.  It will combine theoretical and practical aspects to cover topics, including Failure Mode Effect Analysis (FMEA), criticality analysis, Root Cause Analysis (RCA), risk based inspection, and decision making analysis for production operation activities. The course will go through the methodology of FMEA in production systems by identifying the failure modes of the various parts of a system, the failure causes, the effects that these failures may have on the system and how to avoid the failures and/or mitigate the effects of the failures on the system.

    Day 1

    Failure Mode Effect Analysis (FMEA) Overview and Criticality Analysis 

    • Failure Mode Effect Analysis (FMEA) overview
    • Criticality analysis
    • Qualitative models
    • Semi-quantitative models

    On the first day, participants will get a quick introduction of the Failure Mode Effect Analysis (FMEA) and other methodologies used along with it.  The day will continue on to criticality analysis.  It will cover which system may be more important than another, how criticality analysis is done, and hierarchy assets.  The day will end with a discussion on qualitative models, semi-quantitative models, and the application of production systems.

    Day 2

    FMEA (Cont.)

    • Types of failure mode effect analysis 
    • Where and when to use a FMEA 
    • How to define a failure mode and the effect of the failure 
    • Classification of the failures 

    Day two will focus on FMEA, the different types, the functional approach, physical condition, and when and where to use FMEA.  Input process – output diagram, failure mode, the effect of failure, risk associated to a failure mode, and classifications of failures will also be covered.                 


    Day 3

    Application of FMEA to Subsurface Surface Systems

    • Application of FMEA to subsurface surface systems
    • Case studies

    The focus of day three will be the application of FMEA to subsurface surface systems.  Participants will also get a chance to see several cases related to production operation systems dealing with well tests, ESP, and compressors.


    Day 4

    Root Cause Analysis (RCA) 

    • Basic terms and methodologies
    • Fault tree and fishbone methodologies
    • Streamline methodology

    On day four participants will learn about the Root Cause Analysis (RCA) basic terms and different methodologies in association with FEMA, including failure, functional failure, failure mode, and failure cause.  The day will continue with a discussion on the fault tree, fishbone, and streamline methodologies.

    Day 5

    Risk Based Inspection and Maintenance

    • Risk based inspection 
    • Best practices that sustain world class maintenance 
    • Maintenance options based on FMEA and RCA

    The last day will teach participants about risk based inspection and the association with FMEA to mitigate and monitor the failure mode and the maintenance plan.  The participants will learn about the best practices to sustain world class maintenance and maintenance options.


    Surface facility operation engineers, operation managers, maintenance engineers, and facility and production engineers.
