    HSE Event Reporting Level 1

    Course Summary:

    • You may brush off a near-miss incident at work but that’s the worst way to respond to this type of close call.
    • A near miss event often indicates that there are potential safety hazards in the workplace.
    • To ensure safety at your company, and avoid OSHA-related penalties, all near misses need to be reported.
    Objectives
    • Encourage reporting and recording of all HSE events.
    • All accidents with Serious or greater consequences and High Potential events shall be investigated.
    • The following HSE Light accidents shall be investigated:
    - An injury (medical treatment or first aid)
    - Radiation or explosives (security, exposure to personnel or environmental)
    - Personal security

    • Investigation teams shall include line management, the HSE and/or OI Function, and appropriate internal or external resources, as defined in the Personal and Process Safety Responsibility Matrices.
    • Investigations shall be conducted according to the SLB accident investigation model.
    • Line management shall prioritize remedial actions to prevent a recurrence.
