    The Incident Management and Investigation with Root Cause Analysis course is designed to equip participants with essential skills for effectively investigating workplace incidents. The course covers techniques for collecting complete, accurate, and objective data, identifying root causes, documenting findings, and recommending corrective actions.

    Through discussions, demonstrations, and hands-on exercises, participants will explore investigation and interview methods such as the 5 Whys, Fishbone Diagram, and other proven tools. They will learn to uncover who, what, where, when, why, and how of each incident and analyze the findings to help prevent injuries, property damage, and financial losses

    Pre and Posttest

    Pre- and post-assessment tests will be conducted at the beginning and end of the class to evaluate the participant's level of knowledge and the outcome of the training.

    Course Evaluation

    The course evaluation form will be distributed to all participants at the end of the course to evaluate the course materials, presentation, instructor, training facility, and the training program performance. 

    Daily training progress report

    The instructor will submit a daily training progress report.

    Final Exam

    The final Exam must be passed with a 70% to pass the course.
